Why do we collect this information?

Redditch Borough Council need the personal information you supply on this form so that we can record the cremation.

The legal basis for our collecting and processing this information is public task.

Please note that Redditch Borough Council does not hold the recording of the service itself (see below for more details).

What personal information is collected?

• Your signature
• Your address if you are the minister, organist, funeral director or other person involved in the service.

Who has access to the information?

This information will be accessible to the staff working in the Bereavement Services team at Redditch Borough Council.

Your information will not be shared with or sold to any other service or organisation unless we have a duty to do this under law.

Is the information sent outside the EEA?

This information will not be transferred outside the European Economic Area.

How long is the information kept?

This information will be kept for 15 years, as required by the Cremation Regulations 2008.

Are any automated decisions made using this information?

No decisions on this information are made by automated means.

Who records the service?

Wesley Media (of Yaboo Company Ltd), Kettering, provide the recording service and send us the recording on a USB stick, CD or DVD, and we will send this to the applicant (or the funeral director at the applicant’s request). The service can also be streamed live as a webcast.

Apart from receiving and sending out the USB stick, CD or DVD, Redditch Borough Council does not have access to the recording.
