We need the personal information you supply on this form so that we can organise the open day you plan to attend. We are asking for your email address so that we can contact you if necessary before the event – for example, if the event had to be postponed or cancelled for any reason.

This information will be accessible to the staff working in the Bereavement Services teams at Bromsgrove District Council and Redditch Borough Council.

The information will be kept for one week after the event takes place.
