Activity on Referral

'Activity on Referral' is a county wide scheme whereby GP's and Health Professionals can refer physically inactive people onto a tailor-made exercise programme to assist and support them to make healthier lifestyle choices.

Here in Redditch we are also accepting self-referrals, if you would like to join the scheme and your health condition is not in the list below please just contact us we can have a chat about your health and what we can do to help.

How does it work?

If you feel your health or medical condition would benefit from you becoming more active, you could be referred into the scheme. A qualified instructor will then prescribe an exercise programme for you.  Once your referral has been received we will be in touch to let you know when the next 6 week course is starting.

Who is it for?

To be eligible for the scheme you must be; over the age of 16, inactive, and suffer from a medical condition that would benefit from a programme of exercise. You may qualify for the scheme if you are:

  • Overweight stressed or depressed
  • Type 2 Diabetes
  • High Cholesterol
  • Have a family history of Heart Disease
  • Impaired Strength or Mobility
  • High Blood Pressure
  • Stroke
  • Fibromyalgia
  • Chronic Fatigue Syndrome
  • COPD

Getting Referred

If you think you are eligible for the scheme, then you can contact us about being referred onto the programme, or complete this form and return it to us

Once referred you will be contacted with details of the next 6 week course.  The instructor will carry out some simple health checks, discuss your choice of activities and then write you an exercise programme specific to your goals. The programme will last for 6 weeks and will be under the supervision of trained instructors. During this time you will also have access to our all of our community exercise sessions.   Please follow this link to see what is available.

How much does it cost?

There is a small charge each time you attend the scheme of £2.00.
